In connection with my application for insurance, I hereby consent that Foremost may review my credit report or obtain or use a credit-based insurance score based on information contained in that credit report. I understand that they may use a third party in connection with the development of my insurance score. I further agree that the Company may investigate and secure new consumer reports in evaluating my policy for each future renewal or replacement policy.

We will ask you on the estimate request form if we have your consent to obtain the necessary credit report. If not, we will not be able to provide you with an estimate.
